Hours:
Our physicians
provide care Monday – Friday between the hours of 7:00 am and 6:00 pm.
Each physician has his or her own specific schedule during the week, which may
vary from time to time. Please consult with a receptionist regarding the exact
hours of your own personal physician.

Our office is
located on the fifth floor of the new Medical Office Building / Neuroscience
Institute, which is a new beige brick building located on the Providence Park
Campus at the corner of Grand River and Beck. The building is located on
Providence Parkway, a new road running from Beck Road to Grand River.
It is located behind the new Hospital, which is the large grey and brown
building at the center of the campus.

Our
goal is to address your needs on the phone in an
efficient and accurate manner. We utilize a phone
menu system in our Novi office to direct your
calls to the most appropriate site within the
office. We ask that you make use of the menu choices
to best allow us to serve you.
To assist you in the navigation of this system,
we provide the following summary of our menu options.

Appointment Information:
We offer the following services:
| Physical Exams |
We perform routine annual physicals to determine your general health and guidelines to protect your health into the future. Please call for an appointment at least 1-2 months prior to your desired physical time. |
| Preoperative Evaluations |
We will evaluate your health to optimize your medical condition prior to any planned surgery. Please call for an appointment as soon as you have scheduled your surgery to all time for any necessary testing and treatment. |
| Office Visits |
We will address any long-term medical conditions and discuss any new problems or concerns. |
| Urgent Appointments |
We will address any urgent problems you may have. Please call the office for an appointment which can usually be scheduled for the same or next day. We are more efficient in addressing your needs if you call for an appointment rather than walking into the office without an appointment. In order to address your problem quickly, you may be asked to see any available physician. If you are having a medical emergency or potentially life-threatening condition (eg. chest pain, difficulty breathing, stroke symptoms, etc) please call 911 or go directly to the nearest emergency room. |
| Nurse Visits |
You can schedule an appointment to receive allergy shots, immunizations, flu shots and other services by our clinical staff. |

Fees and Payments:
We accept most major insurances plans. These insurance plans will cover a lot of the services we provide. Any portion of the fees not paid by the insurance company is the responsibility of the patient and must be paid directly. We insist that all payments be made at the time of service. This will avoid billing problems and unnecessary hassle in the future. It is important to realize that every insurance policy has its own level of coverage and list of covered services. We will attempt as best as we can to verify that level of coverage for all patients. It is ultimately, however, the responsibility of the patient to understand his or her own insurance and what it provides. It is also important that you alert us as soon as possible if you have a change in your insurance coverage so that we may update your account. We will happy to assist you in any way we can and we appreciate your cooperation

Hours
Care:
For
medical emergencies, please call 911 or go to
the nearest emergency room. If you need to speak
with the physician on call, please call our
answering service at (248) 584-6109. Please
do not call the physician on call for any non-urgent
matters such as test results, prescription refills,
referral questions or medical issue which can
wait until the next business day.
Hospital Care:

Our physicians provide inpatient hospital care
at Providence Hospital in Southfield. Care for patients admitted to the hospital is provided in conjunction with a team of Hospitalists. In order to provide
the best possible care, the physician making
rounds at the hospital will maintain close contact
with your regular physician regarding your treatment.

Prescription Refills:
We make every effort to write your prescriptions
so that you will have an adequate supply until
your next appointment. To assist us in this,
please bring all of your prescription bottles
with you to your follow-up appointments. It
is important that you do not stop taking your
medications or let your prescriptions run out.
Should you need more medicine prior to your
next appointment, please contact the office
so that we may assist you with your refills.
We provide different ways to request a refill,
whichever suits you best:
(1) Automated Prescription Line on the phone
system (menu option 3)
(2) Online Refill Request
(3) Request a refill from a receptionist
All requests will be processed by the end of
the next business day. We ask that you do not
wait until the last minute to request a refill.

Managed Care Referrals:
"Managed Care" refers to insurances, particularly HMO’s and certain PPO’s,
that involve the use of a Primary Care Provider (PCP) to guide and oversee
your medical care. Our physicians function in the role of PCP for these insurances.
In this system, patients are required to obtain "referrals" and "authorizations"
to see specialists (such as cardiologists, etc.) and to undergo certain medical
testing (such as MRI’s, CT scans, and heart testing, etc). These referrals must
be obtained from your PCP prior to your scheduled appointment. You must notify
our office if you have scheduled an appointment for any medical tests or with a
specialist so that we may contact your insurance to issue a referral. As it may
take up to one week to issue a referral from the insurance company, we ask that
you contact us as soon as possible regarding any upcoming appointments. At this
time, we are unable to issue any referrals for appointments that you have had in
the past. As a result, most specialists will not allow you to keep your
appointment without the necessary paperwork.
Every insurance has different levels
of coverage and different referral procedures. Even the same insurance company
may have different procedures for different patients depending on the level of
coverage. It is the patient’s responsibility to understand the requirements of
their own individual policies. We will attempt to help you in any way possible.
To assist you in this process, we recommend following these steps:

Contact our office if you require
the care of a specialist or require any medical tests. We will review your insurance, determine
if you require authorization, and direct you to the necessary facilities and physicians within
your network. It may be necessary for you to schedule an appointment with your PCP to determine
the nature of your needs. |

Schedule the necessary appointments according to the
information given by our office. |
| 3 |
Contact our office again with the date and time of your
appointment. You may utilize our automated phone line
(Menu Choice # 5) to leave a message with the necessary information
or you may speak with one of our associates. |
| 4 |
Pick up your referral in our office before your
appointment. If your appointment is not within our medical building
we will fax your referral to the necessary site for you.
Because of the unpredictability of the mail,
we do not mail referrals. |
